My friend (male) recently told me when he pisses he also cums sometime I don't know all the time but sometime and i was wondering if that is normal or something that needs medical attention

It's not common. There's some plumbing in us that makes it difficult to piss through a b0ner. Not impossible, but it takes some relaxing while pushing piss out. I don't think I (he) should be concerned as long as he shoots jizm when it's time to do that.
