The 747-400 introduced in 1989 was the first variant with a 2 crew cockpit.

when they realized that if anything goes wrong on a modern airplane there's nothing anyone can do and they are all doomed

They didn't need them any more. Jet engines are much less complicated than the piston engines on older airliners. But it took quite some time to negotiate with the unions to eliminate the positions.
