Likely you have a skin inflammation, bacterial infection. Wash the area with a solution of hydrogen peroxide, or Isopropyl alcohol; let it dry, then apply the heat.

It may be a reaction or just that it heating the area too fast which is causing the skin to feel irritated I would recommend using a different brand
