Depends on the type of port.
An implanted port (also known as a power port, as it can be used for rapid infusion) is surgically located under the skin, typically just below the clavicle or collarbone. Little or no scarring is seen on the skin surface after the incision is healed. I've had mine for 2 years and unless I tell someone, its completely unnoticeable.
An external port. hangs outside the body and can been seen through thin fabrics. Looks like an IV site. Prone to infections.

I imagine it depends on the type of shirt. I had a catheter in my jugular for dialysis. I was able to dress to conceal it.
