All you need to do is tell your new doctor what hospital you went to and what doctor you saw. They will do the rest. If for whatever reason that won't work then call the hospital and request medical records be faxed to your new doctor. If you ask them to be faxed to you they will do it but may charge you.

hmm, you should be able to request it over the phone by calling the hospital where it was done. you doctor should also be able to call the hospital himself and request it to be sent, so sounds like he is being lazy, because even his secretary could call to request it.
otherwise, yes you could go in person also.
i recommend asking the new doc to call and request it to be sent over. they seem to known exactly which department to call. i hate calling and getting transferred to a million different people.

Your Dr. will probably provide it automatically, but you can just ask him for a copy. I have done this many times.
