bthserv 32.dll keeps coming back how to eliminated for good?

I have deleted this backdoor Trojan but it reapears

Update:

It is a trojan as it flags every file that's run as a bad image. Apparently the trojan version of this file is downloaded with lime wire

    Why do you think Bthserv 32.dll is a trojan backdoor ?
